Preparing for appointments

2 min read

A before-and-after checklist for organising questions, travel, practical support and follow-up tasks around an appointment.

Check the details early

Confirm the date, time, address, expected duration and any preparation requested by the provider. Ask about access requirements, interpreters or communication support if needed. Check what documents or information the provider wants you to bring. Keep these in a secure place rather than sending them through a general enquiry form.

Write down what you want to ask

Choose your most important questions and put them somewhere easy to find. You might want to understand what will happen at the appointment, what choices are available or what the next step will be. It is reasonable to ask for unfamiliar language to be explained and for instructions in a format you can use.

Agree on the support person’s role

If a worker or trusted person accompanies you, decide what you want them to help with. Options include organising the journey, finding the reception desk, taking notes with your permission or helping you remember questions. Discuss who will speak and what information can be shared. A support worker helps with practical participation; the appointment provider remains responsible for their professional advice.

Before you leave checklist

  • Appointment time, location and contact details saved.

  • Travel, parking or public transport route checked.

  • Return journey and a little extra time allowed for.

  • Provider-requested documents or preparation ready.

  • My most important questions written down.

  • Communication and access needs discussed.

  • My support person knows what I do and do not want help with.

Capture the next step before leaving

Ask what needs to happen next, who is responsible and when it should happen. If you are unsure about instructions, ask the provider to clarify them. Do not rely on a support worker to interpret clinical instructions or change treatment advice. Keep any professional advice or paperwork somewhere you can find it again.

Follow-up note you can copy

Date and provider: _____. Main points I want to remember: _____. My next action: _____. Who will help, if I want support: _____. Due date or next appointment: _____. Question I still need to ask the provider: _____.

Check travel and costs separately

Confirm worker time, travel, parking and any other costs before agreeing to support. Where funding is involved, check the arrangement rather than assuming the appointment or journey will be covered.
